Medication Management
The Center for Collaborative Health now offers medication services with Board Certified healthcare providers.
Services include medication management, comprehensive treatment planning, consultation, and coordination of care with other healthcare professionals. Our prescribers provide a high-quality service by spending time with each client and building a strong therapeutic alliance. Our clinicians do not rush through appointments, but instead take the time to get to know each client and support them therapeutically at every visit.
We believe in using a collaborative approach to treat our clients at CCH and our medication management service is no different.
At your first visit, you can expect your prescriber to complete a thorough evaluation of you, your health history, and the symptoms you are currently experiencing. This evaluation should last approximately one hour.
Based on this assessment, your prescriber will determine whether or not you might benefit from medication services and explore this option with you. They will also assess whether other services at CCH might be helpful to you such as individual therapy, psychological assessment, nutrition counseling, or lifestyle changes. If medications are recommended and ultimately prescribed to you, your prescriber will then schedule follow-up appointments with you to continue monitoring the effectiveness of your treatment.
Our medication management services are currently available for individuals aged 0 – 25. Adult medication services coming soon.
Insurance Information:
Our medication management services can be billed to insurance or billed directly to the client. CCH is an in-network provider for Aetna, Blue Cross Blue Shield, CIGNA Behavioral Health, Health Partners, Hennepin Health, Medica, Medical Assistance/Medicaid, UnitedHealthcare, PreferredOne, and UCare. We are NOT credentialed with Medicare. If you would like to use your out-of-network benefits with your insurance plan we will provide you with a receipt to submit to your insurance provider. Please contact your insurance company prior to your first session to ensure you are covered for psychiatric services and to understand your deductible/co-pay responsibilities.
